Assistance in Making Follow-Up Appointments

COMPASS helps you find your way to follow-up care.

Because regular check ups are so important in maintaining good health, we may call patients who have not had follow-up care. You may get a call from your physician's office to remind you it is time to come in for a checkup.

If you are admitted to the hospital, someone from Call-A-Nurse may contact you after discharge to determine if you need assistance in making a follow-up appointment. You should be seen by your primary care physician within two weeks of being released from the hospital.
