A care manager may work with you following hospitalization or emergency room visits to verify you have the services needed.

These services may include home health, specialty clinics (e.g., wound care clinic, nutrition or diabetes education) and further evaluation by specialists. Your care is coordinated by your primary care physician.

Discuss with your physician the benefit by a referral to the COMPASS disease management program.
